Targeted Case Management

What is Case Management?

Upon intake HFS case managers will conduct a face to face contact with the child to do an initial assessment of immediate needs. When it is appropriate HFS case managers will also meet with family members, other natural supports and providers currently involved with the family. A plan of care will then be developed with the child and family team to determine what services and resources are necessary to meet the identified needs and to determine how they might be most appropriately delivered to maintain current service delivery and to resolve gaps in service so that comprehensive care is attained. Once these needs are established case managers will then take an inventory of existing strengths and resources and begin the process of child and family empowerment through an effort of coordination and advocacy. Whenever possible HFS case managers will empower the child and family team to take action to meet their identified needs.

Who is eligible for Case Management?

Eligibility for this service is determined by the state and the managed health care company they have hired to administer the service. They will authorize services if the child’s behavior has caused significant problems in their home, school or community and as such the family needs assistance in the identification and coordination of community services. The child referred must have Maine Care, be between 0-20 years old and have a DSM IV diagnosis or a DC 0-3 diagnosis which has lasted or is expected to last for one year.

HFS case managers work closely with the child and family to ensure that his/her/their desires and needs are identified and addressed. HFS case managers provide assistance in the following areas:

• Education and Employment
• Social Empowerment
• Family Empowerment
• Scheduling and documentation of appointments and meetings
• Utilization of natural and community supports
• Maintaining a relationship with the Department of Health and Human Services
• Developing relationships with community provider agencies and services
• Developing a knowledge of community resources

How we get Case Management Services

You may contact us directly to make a referral or you may request that your caseworker or other provider contact us on your behalf. We will forward your referral to Central Enrollment (MaineCare’s referral service). Central Enrollment will contact you by mail of all approvals and denials.
