BHP/Case Manager - Rockport

Program: Community Services Program – Rockport

Position: BHP/Case Manager (Full-time)

Job Description: This role is a multifaceted, case management role within the Harbor Family Services, Community Based Services program. The BHP/CM is the primary link for outside services as well as to provide hands on services for the family. The BHP/CM will work closely with State agencies and other professionals, representing Harbor Family Services in the community at all times. The BHP/CM will also work closely with the Director of Community Based Services assisting in the development of new programming.

Qualifications: Bachelor’s Degree
1 – 3 years experience with child/adolescents/families or case management experience strongly preferred
Valid Driver’s License and car insurance required.

Salary Scale: Based upon education and experience
